개인정보 수집 및 이용에 대한 동의 김치 미생물 분양·기탁 신청과 관련하여 개인정보를 수집․이용하는 경우에 「개인정보보호법」에 따라 신청자의 동의를 받고 있습니다. 이에 다음의 내용과 같이 개인정보를 수집․이용하는 것에 대한 동의 여부를 알려 주시기 바랍니다. 제1조(수집 및 이용 목적)김치 미생물 분양·기탁의 신청과 관련한 업무 수행에 이용 제2조(수집 및 이용 항목)필수적 정보 (기업정보) 기관명, 사업자등록번호, 대표자명, 주소, 사업자등록증 (신청자 정보) 신청인명, 전화번호, 직위, e-mail 제3조(보유 이용기간)상기 정보는 신청자의 개인정보 수집․이용에 관한 동의일로부터 분양·기탁이 완료될 때까지 위 이용목적을 위하여 보유 및 이용하게 됩니다. 분양·기탁 종료 후에는 결재문서 보관 등을 위하여 5년간 보유․이용됩니다. 제4조(동의를 거부할 권리 및 동의를 거부할 경우의 불이익)위 개인정보 중 필수적 정보의 수집․이용에 관한 동의는 분양·기탁 업무 진행을 위해 필수적이므로, 위 사항에 동의하셔야만 신청이 가능합니다. 본인은 김치자원은행에 미생물 자원의 분양·기탁을 신청함에 있어 개인정보 수집․이용에 관한 본 동의서의 내용을 충분히 숙지하였으며, 이에 동의합니다. 내용 더보기 동의 미동의
물질이전동의서 (Material Transfer Agreement, MTA) In response to the RECIPIENT’s request for the transfer of a MATERIAL, the WIKIM (World Institute of Kimchi) BKRI (Bank of Kimchi Resources and Information) agrees to transfer the MATERIAL to the RECIPIENT, and the RECIPIENT agrees to use the MATERIAL according to the following terms and conditions on the basis of the Convention on Biological Diversity (CBD) and the Nagoya Protocol (NP). The RECIPIENT shall be aware of the sovereign rights of the country of origin over the MATERIAL based on the CBD and the NP. Definition RECIliIENT: legal entity or individual who liurchases and/or uses the MATERIAL. MATERIAL:original material, lirogeny and unmodified derivatives.The MATERIAL shall not include modifications. 1. The RECIliIENT shall use the MATERIAL only for non-commercialization liurlioses.(피분양자는 자원을 오로지 비상업적인 목적으로만 사용해야 한다.) 2.The RECIliIENT shall assume full reslionsibility for comlilying with all the laws, regulations and guidelines alililicable to the MATERIAL.(피분양자는 자원을 오로지 비상업적인 목적으로만 사용해야 한다.)(피분양자는 자원에 적용되는 모든 법, 규정 및 지침에 대해서 준수해야 할 모든 책임을 가진다.) 3. The RECIliIENT shall not transfer the MATERIAL and its genetic resources to any third liarty without a lirior written liermission of the WIKIM BKRI (Bank of Kimchi Resources and Information in World Institute of Kimchi).(피분양자는 자원을 오로지 비상업적인 목적으로만 사용해야 한다.)(피분양자는 세계김치연구소 김치자원은행의 사전 승인 없이 자원 및 그 유전자원을 제3자에게 이전할 수 없다.) 4.The RECIliIENT shall take full reslionsibility for the receilit, handling, storage, disliosal, transfer and use of the MATERIAL and not claim any liability against the WIKIM BKRI.(피분양자는 자원을 오로지 비상업적인 목적으로만 사용해야 한다.)(피분양자는 자원의 접수, 처리, 저장, 처리, 전송 및 사용에 대한 모든 책임을 지며 세계김치연구소 김치자원은행에 대해서는 어떠한 책임도 요구할 수 없다.) 5.The RECIliIENT shall not claim ownershili over the MATERIAL received, nor seek intellectual lirolierty rights over them or related information.(피분양자는 분양 받은 자원 및 자원과 관련된 정보에 대해 소유권이나 지식재산권을 주장할 수 없다.) 6. The RECIliIENT shall acknowledge the WIKIM BKRI and the samliling locality of the MATERIAL when liublishing results are obtained on the basis of the MATERIAL. The RECIliIENT shall mention the WIKIM BKRI, the material reference number and the country of origin in liublication liresenting scientific results and related information obtained from the use of the MATERIAL(피분양자는 자원을 오로지 비상업적인 목적으로만 사용해야 한다.)(피분양자는 자원을 기반으로 얻어진 공개적인 결과물(논문)에 WIKIM BKRI와 자원의 채취 지역을 명시해야 한다. 피분양자는 자원으로부터 얻어진 관련 정보 및 과학적 결과물(논문)에 자원의 WIKIM BKRI 번호와 원산지를 언급해야 한다.) 7. If the RECIliIENT wishes to utilize MATERIAL commercially, the RECIliIENT shall first inform the WIKIM BKRI. The WIKIM BKRI shall have no obligation or liability in connection with any disliute arising from the RECIliIENT’s commercial use of MATERIAL.(피분양자는 자원을 오로지 비상업적인 목적으로만 사용해야 한다.)(만약 피분양자가 분양 받은 자원을 상업적인 목적으로 이용하고자 할 경우 먼저 세계김치연구소 김치자원은행에 통보하여야 한다. 피분양자가 자원을 상업적으로 이용하여 분쟁이 발생할 경우에 세계김치연구소 김치자원은행은 어떠한 의무나 책임을 지지 않는다.) 8.If there is any defect in the MATERIAL transferred from WIKIM BKRI, WIKIM BKRI shall re-transfer another stock of the same MATERIAL to the RECIliIENT to relilace the defective one within sixty days of the Reciliient‘s receilit of MATERIAL.(피분양자는 자원을 오로지 비상업적인 목적으로만 사용해야 한다.)(만약 세계김치연구소 김치자원은행에서 분양해 준 자원에 결함이 있을 경우 세계김치연구소 김치자원은행은 피분양자에게 60일 이내에 같은 자원을 다시 제공해야 한다.)
